ABSTRACT

ABSTRACT Objective To present the frequency and species diversity of non-tuberculous mycobacteria, estimate the prevalence of non-tuberculous mycobacterial pulmonary disease, describe the epidemiological profile, and determine the follow-up of patients with non-tuberculous mycobacterial pulmonary disease living in a region with a high burden of tuberculosis. Methods This a retrospective cohort observational study using data records obtained from the Instituto Adolfo Lutz - Santos and from the São Paulo Sistema de Vigilância de Tuberculose do Estado de São Paulo in the period between 2000 and 2009. The studied variables were: socio-demographic characteristics, current and past history of tuberculosis, aspects related to diagnosis, and treatment and associated diseases. Results We included 319 non-tuberculous mycobacteria isolates in the study, corresponding to 257 patients. The species Mycobacterium kansasii (28.5%) and Mycobacterium fortuitum (16.6%) presented the higher occurrence. In 10.9% (24) of the patients, there was a criterion for confirming a case of pulmonary disease due to non-tuberculous mycobacteria. In relation to gender and age, male and individuals over 50 years old were the most frequent. Considering the confirmed cases, 47.8% had a past history of tuberculosis. Conclusion The lack of information about the cases is evident, since pulmonary disease due to non-tuberculous mycobacteria is not mandatory. The therapeutic regimen according to the identified species is fundamental for success in combating the infections caused by non-tuberculous mycobacteria. Besides that, information about the regional epidemiology of pulmonary disease caused by non-tuberculous mycobacteria and the search for associations with other comorbidities are important to establish the correct treatment. In order to improve surveillance of pulmonary diseases by non-tuberculous mycobacteria, we suggest the implantation of a sentinel surveillance and of population-based studies.

OBJECTIVE: To report initial experience from the use of extracorporeal membrane oxygenation (ECMO) in patients who received lung transplantation. METHODS: Retrospective study of a single tertiary center in the Brazilian state of São Paulo, a national reference in lung transplantation, based on the prospective collection of data from electronic medical records. The period analyzed extended from January 2009 (beginning of the program) until December 2018. RESULTS: A total of 75 lung transplants were performed, with ECMO used in 8 (10.7%) cases. Of the patients, 4 (50%) were female. The mean age was 46.4±14.3 years. The causes of the end-stage lung disease that led to transplantation were pulmonary arterial hypertension in 3 (37.5%) patients, bronchiectasis in 2 (25%) patients, pulmonary fibrosis in 2 (25%) patients, and pulmonary emphysema in 1 (12.5%) patient. In our series, 7 (87.5%) cases were sequential bilateral transplantations. Prioritization was necessary in 4 (50%) patients, and in 1 patient, ECMO was used as a bridge to transplantation. The ECMO route was central in 4 (50%), peripheral venovenous in 2 (25%) and peripheral venoarterial in 2 (25%) patients. The mean length of the intensive care unit (ICU) stay was 14±7.5 days and of the hospital stay was 34.1±34.2 days. The mean ECMO duration was 9.3±6.6 days with a 50% decannulation rate. Three patients were discharged (37.5%). CONCLUSION: Lung transplantation requires complex treatment, and ECMO has allowed extending the indications for transplantation and provided adjuvant support in the clinical management of these patients.

ABSTRACT Objective: Bone disease is a common comorbidity in patients with cystic fibrosis (CF). We sought to determine risk factors and identify potential biochemical markers for CF-related bone disease (CFBD) in a unique cohort of CF patients with end-stage lung disease undergoing lung transplantation (LTx) evaluation. Methods: All of the CF patients who were evaluated for LTx at our center between November of 1992 and December of 2010 were included in the study. Clinical data and biochemical markers of bone turnover, as well as bone mineral density (BMD) at the lumbar spine and femoral neck, were evaluated. Spearman's rho and multivariate logistic regression analysis were used. Results: A total of 102 adult CF patients were evaluated. The mean age was 28.1 years (95% CI: 26.7-29.5), and the mean body mass index was 17.5 kg/m2 (95% CI: 17.2-18.2). Mean T-scores were −2.3 and −1.9 at the lumbar spine and femoral neck, respectively, being lower in males than in females (−2.7 vs. −2.0 at the lumbar spine and −2.2 vs. −1.7 at the femoral neck). Overall, 52% had a T-score of < −2.5 at either skeletal site. The homozygous Phe508del genotype was found in 57% of patients without osteoporosis and in 60% of those with low BMD. Mean T-scores were not particularly low in patients with severe CFTR mutations. Although the BMI correlated with T-scores at the femoral neck and lumbar spine, serum 25-hydroxyvitamin D and parathyroid hormone levels did not. Conclusions: CFBD is common in CF patients with end-stage lung disease, particularly in males and patients with a low BMI. It appears that CF mutation status does not correlate with CFBD. In addition, it appears that low BMD does not correlate with other risk factors or biochemical parameters. The prevalence of CFBD appears to have recently decreased, most likely reflecting increased efforts at earlier diagnosis and treatment.


RESUMO Objetivo: A doença óssea é uma comorbidade comum em pacientes com fibrose cística (FC). Nosso objetivo foi determinar os fatores de risco e identificar possíveis marcadores bioquímicos de doença óssea relacionada à FC (DOFC) em uma coorte única de pacientes com FC e doença pulmonar terminal submetidos a avaliação para transplante de pulmão (TxP). Métodos: Todos os pacientes com FC avaliados para TxP em nosso centro entre novembro de 1992 e dezembro de 2010 foram incluídos no estudo. Foram avaliados dados clínicos e marcadores bioquímicos de remodelação óssea, bem como a densidade mineral óssea (DMO) na coluna lombar e colo do fêmur. Foram usados rô de Spearman e análise de regressão logística multivariada. Resultados: Foram avaliados 102 pacientes adultos com FC. A média de idade foi de 28,1 anos (IC95%: 26,7-29,5), e a média do índice de massa corporal foi de 17,5 kg/m2 (IC95%: 17,2-18,2). A média do escore T foi de −2,3 e −1,9 na coluna lombar e colo do fêmur, respectivamente, sendo menor nos homens que nas mulheres (−2,7 vs. −2,0 na coluna lombar e −2,2 vs. −1,7 no colo do fêmur). No geral, 52% apresentaram escore T < −2,5 em um dos dois sítios esqueléticos. O genótipo homozigoto para Phe508del foi encontrado em 57% dos pacientes sem osteoporose e em 60% daqueles com DMO baixa. A média do escore T não foi particularmente baixa em pacientes com mutações graves do gene CFTR. Embora o IMC tenha se correlacionado com o escore T no colo do fêmur e coluna lombar, os níveis séricos de 25-hidroxivitamina D e paratormônio não o fizeram. Conclusões: A DOFC é comum em pacientes com FC e doença pulmonar terminal, particularmente em homens e pacientes com IMC baixo. O estado de mutação da FC aparentemente não se correlaciona com a DOFC. Além disso, aparentemente não há correlação entre DMO baixa e outros fatores de risco ou parâmetros bioquímicos. A prevalência de DOFC parece ter diminuído recentemente, o que provavelmente é reflexo do aumento dos esforços para antecipar o diagnóstico e tratamento.

Resumo Este estudo teve como objetivo descrever os casos de internações hospitalares e impacto financeiro da tuberculose pulmonar no estado da Bahia, Brasil, entre o período de 2012 e 2016. É um estudo epidemiológico, descritivo transversal, utilizando dados do Departamento de Ciência da Computação do Sistema Único de Saúde. As variáveis coletadas foram: sexo, faixa etária, valores dos serviços hospitalares e raça, relatadas no período de 2012 e 2016. Os casos de internações hospitalares foram selecionados de acordo com o capítulo I da 10ª Classificação Estatística Internacional de Doenças e Problemas de Saúde, incluindo as categorias entre A15.0 e A15.3. Entre os resultados, constatou-se que no período estudado, foram registrados 5.593 casos de internações hospitalares por tuberculose pulmonar no estado da Bahia, o que corresponde a 0,14% do total de internações hospitalares. A maior prevalência ocorreu na macro região oriental (n = 4.623), entre os homens (n = 4.079), entre 40 e 49 anos (n = 1.361) e cor / raça ignorada (n = 5.176). A tuberculose pulmonar gerou um impacto financeiro na Bahia de mais de 7 milhões de reais no período estudado e a macrorregião oriental gerou maiores gastos para o estado, correspondendo a 91,70% do total de custos hospitalares. Conclui-se que na macro região oriental, o modo de distribuição dos casos de internações hospitalares por tuberculose pulmonar faz da macrorregião uma prioridade para as ações de controle e prevenção da patologia.


Resumen Este estudio tuvo como objetivo describir los casos de internamientos hospitalarios y el impacto financiero por tuberculosis pulmonar en el estado de Bahía, Brasil entre el periodo 2012 y 2016. Es un estudio epidemiológico, descriptivo y transversal, utilizando los datos obtenidos en el Departamento de Informática del Sistema Único de Salud. Las variables recolectadas fueron: sexo, grupo de edad, valores de los servicios hospitalarios y raza, notificados en el período de 2012 y 2016. Los casos de internamientos hospitalarios han sido seleccionados de acuerdo con el capítulo I de la 10ª Clasificación Estadística Internacional de Enfermedades y Problemas de Salud, comprendiendo las categorías entre A15.0 y A15.3. Entre los resultados se obtuvo que en el período estudiado, se han registrado 5.593 casos de internamientos hospitalarios por tuberculosis pulmonar en el estado de Bahía, que corresponde a 0,14% del total de las hospitalizaciones. La mayor prevalencia ocurrió en la macro región Este (n=4.623), entre personas de sexo masculino (n=4.079), de grupo etario entre 40 y 49 años (n=1.361) y color/raza ignorada (n=5.176). La tuberculosis pulmonar ha generado en Bahía un impacto financiero superior a 7 millones de reales en el período estudiado y la macro región Este ha generado mayor gasto al estado, correspondiendo a 91,70% del total de los costos hospitalarios. Se concluye que en la macro región Este, el modo de distribución de los casos de internamientos hospitalarios por tuberculosis pulmonar convierte la macro región como prioritaria para las acciones de control y prevención de la patología.


Abstract The purpose of this study was to describe the cases of hospital admissions and the financial impact of pulmonary tuberculosis in the state of Bahia, Brazil between 2012 and 2016. It is an epidemiological, descriptive and cross-sectional study, using the data obtained in the Department of Information Technology of the Unified Health System. The variables collected were: sex, age group, values of hospital services and race, reported in the period of 2012 and 2016. The cases of hospital admissions have been selected in accordance with chapter I of the 10th International Statistical Classification of Diseases and Health Problems, including the categories between A15.0 and A15.3. Among the results, it was found that in the period studied, 5,593 cases of hospital admissions for pulmonary tuberculosis in the state of Bahia were registered, which corresponds to 0.14% of the total number of hospitalizations. The highest prevalence occurred in the Eastern macro region (n = 4,623), between male people (n = 4,079), between 40 and 49 years old (n = 1,361) and color / race ignored (n = 5,176). Pulmonary tuberculosis has generated a financial impact in Bahia of more than 7 million reales in the period studied and the Eastern macro region has generated greater spending to the state, corresponding to 91.70% of total hospital costs. It is concluded that in the Eastern macro region, the mode of distribution of cases of hospital admissions for pulmonary tuberculosis makes the macro region a priority for the actions of control and prevention of the pathology.

Resumen Objetivo: Determinar la frecuencia, tipo y predictores de complicaciones pleuropulmonares en los primeros 30 días de postoperatorio de pacientes intervenidos de cirugía cardiovascular sin apoyo de circulación extracorpórea. Métodos: Se realizó un estudio de cohorte retrospectivo durante el periodo comprendido del 1 de enero de 2013 al 31 de diciembre 2014. Incluyó a todos los pacientes portadores de cardiopatías congénitas intervenidos de cirugía cardiaca con abordaje esternal o torácico, sin soporte de circulación extracorpórea con ingreso registrado a Unidad de Cuidados Intensivos del Hospital de Pediatría del Centro Médico Nacional Siglo XXI, IMSS. Se cuantificó la frecuencia de eventos de las complicaciones pleuropulmonares y se realizó un análisis multivariado de regresión logística para identificar los factores de riesgo asociados a complicaciones pleuropulmonares, calculándose odds ratio (OR) e intervalos de confianza al 95% (IC 95%). Resultados: Se incluyeron un total de 139 pacientes, en los cuales la frecuencia de complicaciones pleuropulmonares fue del 42.4% (n = 59), y los tipos más frecuentes fueron atelectasia (28 eventos), neumonía asociada a ventilador (24 eventos), neumotórax (20 eventos), pudiéndose encontrar más de una complicación por paciente. Los predictores más significativos de complicaciones pleuropulmonares fueron las cardiopatías congénitas cianógenas (OR = 3.58; IC 95%: 1.10-7.50; p = 0.001), el abordaje por toracotomía (OR = 1.46; IC 95%: 1.18-1.12; p = 0.008) y el evento quirúrgico realizado de urgencia (OR = 3.46; IC 95%: 1.51-7.95; p = 0.002). Conclusiones: La principal complicación pleuropulmonar fue la atelectasia lo cual concuerda con lo reportado en la literatura internacional. Los pacientes que presenten alguno de los predictores identificados en el presente estudio deben ser monitorizados de manera especial para prevenir, detectar y/o tratar oportunamente las complicaciones pleuropulmonares tras cirugía cardiaca.


Abstract Objective: To determine the frequency and type of pleuropulmonary complications and their predictors in the first thirty postoperative days of patients undergoing surgery without cardiopulmonary bypass. Methods: A retrospective cohort study was carried out between January 2013 and December 2014. It included all patients with congenital heart disease who underwent cardiac surgery using a sternal or thoracic approach, without cardiopulmonary bypass with a registered admission to a Neonatal or Paediatric Intensive Care. The frequency of events of pleuropulmonary complications and logistic regression analysis was performed, and the adjusted odds ratio (OR) and confidence intervals at 95% (95% CI) were calculated. Results: A total of 139 patients were included. The frequency of pleuropulmonary complications was 42.4% (N = 59), and the most frequent types were atelectasis (28 events), ventilator-associated pneumonia (24 events), pneumothorax (20 events), with more than one complication per patient occasionally being found. Significant risk factors were cyanogenic congenital heart disease (OR = 3.58, 95% CI: 1.10-7.50, P =.001), thoracotomy approach (OR = 1.46, 95% CI: 1.18-1.12, P = .008), and an emergency surgical event (OR = 3.46, 95% CI: 1.51-7.95, P = .002). Conclusions: The main pleuropulmonary complication was atelectasis, which is consistent with that reported in the international literature. Patients with any of the predictors identified in the present study should be closely monitored in order to prevent, detect and/or treat pleuropulmonary complications in a timely manner after cardiac surgery.

ABSTRACT Objective: The incidence of lung disease caused by nontuberculous mycobacteria (NTM) has been increasing worldwide. In Brazil, there are few studies about nontuberculous mycobacterial lung disease (NTMLD), and its prevalence is yet to be known. Our objective was to determine the specific etiology of the disease in the state of Rio Grande do Sul, Brazil, as well as the frequency and diversity of NTM species in our sample of patients. Methods: This is a retrospective analysis of the medical records of patients diagnosed with NTMLD treated in a referral center located in the city of Porto Alegre, Brazil, between 2003 and 2013. Results: Our sample comprised 100 patients. The most prevalent NTM species were Mycobacterium avium complex (MAC), in 35% of the cases; M. kansasii, in 17%; and M. abscessus, in 12%. A total of 85 patients had received previous treatment for tuberculosis. Associated conditions included structural abnormalities in the lungs, such as bronchiectasis, in 23% of the cases; COPD, in 17%; and immunosuppressive conditions, such as AIDS, in 24%. Conclusions: MAC and M. kansasii were the most prevalent species involved in NTMLD in the state, similarly to what occurs in other regions of Brazil. Data on regional epidemiology of NTMLD, its specific etiology, and associated conditions are essential to establish appropriate treatment, since each species requires specific regimens. Most patients with NTMLD had received previous tuberculosis treatment, which might lead to development of resistance and late diagnosis.


RESUMO Objetivo: A incidência de doença pulmonar causada por micobactérias não tuberculosas (MNT) tem aumentado em todo o mundo. No Brasil, há poucos estudos sobre doença pulmonar por MNT, e sua prevalência ainda não é conhecida. Nosso objetivo foi determinar a etiologia específica da doença no estado do Rio Grande do Sul, bem como a frequência e a diversidade das espécies de MNT em nossa amostra de pacientes. Métodos: Análise retrospectiva dos prontuários de pacientes diagnosticados com doença pulmonar por MNT atendidos em um centro de referência localizado na cidade de Porto Alegre, RS, entre 2003 e 2013. Resultados: Nossa amostra foi composta por 100 pacientes. As espécies de MNT mais prevalentes foram Mycobacterium avium complex (MAC, complexo M. avium), em 35% dos casos; M. kansasii, em 17%; e M. abscessus, em 12%. Um total de 85 pacientes havia feito tratamento anterior para tuberculose. Condições associadas incluíram anormalidades estruturais nos pulmões, como bronquiectasias, em 23% dos casos; DPOC, em 17%; e condições imunossupressoras, como AIDS, em 24%. Conclusões: MAC e M. kansasii foram as espécies mais prevalentes envolvidas na doença pulmonar por MNT no estado, à semelhança do que ocorre em outras regiões do Brasil. Dados sobre a epidemiologia regional da doença pulmonar por MNT, sua etiologia específica e condições associadas são fundamentais para se estabelecer um tratamento adequado, já que cada espécie requer um esquema específico. A maioria dos pacientes com doença pulmonar por MNT havia feito tratamento anterior para tuberculose, o que pode levar a desenvolvimento de resistência e diagnóstico tardio.

Se realizó un estudio descriptivo y transversal para caracterizar la población de pacientes con tuberculosis del municipio de Santiago de Cuba, durante los trienios 2008-2010 y 20142016, así como el análisis gráfico de la tendencia de la enfermedad. La población estuvo conformada por 101 casos nuevos con diagnóstico confirmado. Se calcularon las tasas de incidencia y la diferencia porcentual de tasas. Se estimaron porcentajes superiores de afectados en las edades jóvenes e intermedias de la vida (25-34 y 35-44 años) con 21,8 y 22,8 por ciento en el primer trienio, así como 20,7 y 18,2 por ciento en el segundo, respectivamente; aunque en ambos periodos, los mayores riesgos se desplazaron hacia las edades más envejecidas. En ambos trienios predominó el sexo masculino (79,2 y 77,9 por ciento, respectivamente), así como los riesgos de enfermar superiores en el primero con 11,3 casos por cada 100 mil habitantes de dicho sexo. La tendencia de la enfermedad exhibió una progresiva disminución de los pacientes, aunque de forma lenta e irregular


A descriptive and cross-sectional study to characterize the population of patients with tuberculosis in Santiago de Cuba municipality, was carried out during 2008-2010 and 2014-2016, as well as the graphic analysis of the disease tendency. The population was formed by 101 new cases with confirmed diagnosis. The incidence rates and the rates percentage difference were calculated. Higher percentages of affected patients in the young and intermediate ages of life were considered (25-34 and 35-44 years) with 21.8 and 22.8 percent in the first triennium, as well as 20.7 and 18.2 percent in the second, respectively; although in both periods, the highest risks moved toward the most aged. In both trienniums the male sex prevailed (79.2 and 77.9 percent, respectively), as well as the risks of getting sick were higher in the first one with 11.3 cases every 100 thousand inhabitants of this sex. The disease tendency showed a progressive decrease of the patients, although in a slow and irregular way

RESUMO Objetivos: avaliar a incidência de complicações pulmonares pós-operatórias em pacientes submetidos à cirurgias de tórax e abdome e os principais fatores envolvidos. Métodos: estudo analítico observacional prospectivo dos pacientes submetidos à cirurgias de tórax e abdome no Hospital Santo Antônio de Blumenau, SC. Os dados foram coletados dos prontuários eletrônicos e através de entrevistas estruturadas com os pacientes. Foram avaliados dados relativos às características dos pacientes e da cirurgia. A variável de desfecho foi a ocorrência de complicações pulmonares pós-operatórias. Resultados: foram estudados 314 pacientes, 65,6% do sexo feminino, com média de idade de 46,61 anos, 51,6% classificados como ASA II. Cirurgias por vídeo foram realizadas em 55,7% dos casos, abdominais em 85,4% e 61,5% dos procedimentos foram classificados como potencialmente contaminadas e de porte médio. O tempo médio de cirurgia foi de 126,65 minutos e os pacientes ficaram internados em média por 2,59 dias. A incidência de complicações pulmonares pós-operatórias foi de 11,5%. As complicações mais comuns foram a insuficiência respiratória, o derrame pleural e a pneumonia. Os fatores de risco mais importantes para estas complicações foram diabetes, internação hospitalar por mais de cinco dias e presença de doença pulmonar prévia. Os pacientes submetidos às cirurgias por vídeo apresentaram menor incidência de complicações. Conclusão: as complicações pulmonares pós-operatórias são frequentes e os fatores associados a maior risco foram diabetes, internação prolongada e presença de doença pulmonar prévia.


ABSTRACT Objective: to determine the incidence of postoperative pulmonary complications in patients undergoing abdominal and thoracic surgery and investigate the risk factors for their occurrence. Methods: we conducted a prospective, observational and analytical study with all the patients undergoing thoracic and abdominal surgeries at the Santo Antônio Hospital in Blumenau, SC. We collected data from medical records and structured interviews with patients, regarding the characteristics of patients and procedures. The outcome variable was the occurrence of postoperative pulmonary complications in the medical record. Results: we studied 314 patients, 65.6% female, with an average age of 46.61 years. Of the sample, 51.6% was ASA II, 55.7% of the surgeries were performed by laparoscopy or thoracoscopy, 85.4% were in the abdomen, and 61.5% were potentially contaminated and of medium-scale procedures. The average time of surgery was 126.65 minutes and the average length of hospital stay was 2.59 days. The incidence of postoperative pulmonary complications was 11.5%. The most common complications were respiratory failure, pleural effusion and pneumonia. The most important risk factor were diabetes, hospitalization for more than five days and the presence of pulmonary disease. Patients operated by laparoscopy or thoracoscopy had fewer complications. Conclusion: postoperative pulmonary complications are frequent and factors associated with greatest risk were diabetes, prolonged hospitalization and presence of previous lung disease.

ABSTRACT

Objective: To determine the frequency of post-operative pulmonary complications [PPCs] after cardio-pulmonary bypass and association of pre-operative and intraoperative risk factors with incidence of PPCs


Methods: This study was an observational analysis of five hundred and seventeen [517] patients who underwent cardiac surgery using cardiopulmonary bypass. Incidence of PPCs and risk factors of PPCs were noted. Logistic regression was applied to determine the association of pre-operative and intraoperative risk factors with incidence of PPCs


Results: Post-operative pulmonary complications occurred in 32 [6.2%] patients. Most common postoperative pulmonary complication was atelectasis that occurred in 20 [3.86%] patients, respiratory failure in 8 [1.54%] patients, pneumonia in 3 [0.58%] patients and acute respiratory distress syndrome in 1 [0.19%] patients. The main risk factor of PPCs were advance age > 60 years [odds ratio 4.16 [1.99-8.67], p-value <0.001], prolonged CPB time > 120 minutes [odds ratio 3.62 [1.46-8.97] p-value 0.003], pre-op pulmonary hypertension [odds ratio 2.60 [1.18-5.73] p-value 0.016] and intraoperative phrenic nerve injury [odds ratio 7.06 [1.73-28.74], p-value 0.002]. Operative mortality was 9.4% in patients with PPCs and 1.0% in patients without PPCs [p-value 0.01]


Conclusion: The incidence of post-operative pulmonary complications was 6.2% in this study. Advanced age [age > 60 years], prolonged CPB time [CPB time > 120 minutes], pre-op pulmonary hypertension and intraoperative phrenic nerve injury are independent risk factors of PPCs after surgery

ABSTRACT

Objective. To determine whether disease predicts weight loss in population-based studies, as this may confound the relationship between weight and mortality. Materials and methods. We used longitudinal data from the Costa Rican Longevity and Healthy Aging Study (CRELES) and the English Longitudinal Study of Ageing (ELSA). We defined two overlapping outcomes of measured weight loss between waves: >1.0 point of body mass index (BMI) and >2.0 BMI points. Logistic regression models estimated the associations with disease, adjusting for age (range 52-79), sex, smoking, and initial BMI. Results. In ELSA, onset of diabetes, cancer, or lung disease is associated with loss >2.0 points (respectively, OR=2.25 [95%CI: 1.34-3.80]; OR=2.70 [95%CI: 1.49-4.89]; OR=1.82 [95%CI: 1.02-3.26]). In CRELES, disease-onset reports are not associated with weight loss at 5% significance, but statistical power to detect associations is poor. Conclusion. Although it is known that some diseases cause weight loss, at the population level these associations vary considerably across samples.


Objetivo. Determinar si las enfermedades predicen pérdida de peso a partir de encuestas poblacionales, debido a que esto podría confundir la relación entre peso y mortalidad. Material y métodos. Se utilizaron datos longitudinales de Costa Rica: Estudio de Longevidad y Envejecimiento Saludable (CRELES) y Estudio Longitudinal de Envejecimiento en Inglaterra (ELSA, por sus siglas en inglés). Se definieron dos indicadores de resultado no excluyentes de pérdida de peso entre rondas: >1.0 punto de índice de masa corporal (IMC) y >2.0 puntos de IMC. Las asociaciones de interés se estimaron con modelos de regresión logística, con controles para la edad (rango 52-79), sexo, tabaquismo actual e IMC inicial. Resultados. En el ELSA, la incidencia de diabetes, cáncer o enfermedad pulmonar está asociada con pérdida de >2.0 puntos de IMC (respectivamente: OR=2.25 [IC95%: 1.34-3.80]; OR=2.70 [IC95%: 1.49-4.89]; OR=1.82 [IC95%: 1.02-3.26]). En el CRELES, el reporte de diagnóstico de enfermedades no muestra asociación significativa a 5% con pérdida de peso, pero el poder estadístico de la muestra para detectar asociaciones es limitado. Conclusión. Aunque es conocido que ciertas enfermedades causan pérdida de peso, estas asociaciones a nivel poblacional varían considerablemente entre encuestas.

AbstractObjective: To determine whether simple diagnostic methods can yield relevant disease information in patients with rheumatoid arthritis (RA).Methods: Patients with RA were randomly selected for inclusion in a cross-sectional study involving clinical evaluation of pulmonary function, including pulse oximetry (determination of SpO2, at rest), chest X-ray, and spirometry.Results: A total of 246 RA patients underwent complete assessments. Half of the patients in our sample reported a history of smoking. Spirometry was abnormal in 30% of the patients; the chest X-ray was abnormal in 45%; and the SpO2 was abnormal in 13%. Normal chest X-ray, spirometry, and SpO2 were observed simultaneously in only 41% of the RA patients. A history of smoking was associated with abnormal spirometry findings, including evidence of obstructive or restrictive lung disease, and with abnormal chest X-ray findings, as well as with an interstitial pattern on the chest X-ray. Comparing the patients in whom all test results were normal (n = 101) with those in whom abnormal test results were obtained (n = 145), we found a statistically significant difference between the two groups, in terms of age and smoking status. Notably, there were signs of airway disease in nearly half of the patients with minimal or no history of tobacco smoke exposure.Conclusions: Pulmonary involvement in RA can be identified through the use of a combination of diagnostic methods that are simple, safe, and inexpensive. Our results lead us to suggest that RA patients with signs of lung involvement should be screened for lung abnormalities, even if presenting with no respiratory symptoms.


ResumoObjetivo: Determinar se métodos diagnósticos de baixa complexidade podem fornecer informações relevantes sobre doença pulmonar em pacientes com artrite reumatoide (AR).Métodos: Pacientes com AR foram selecionados aleatoriamente para um estudo transversal envolvendo avaliação clínica pulmonar, oximetria de pulso (SpO2) em repouso, radiografia de tórax e espirometria.Resultados: Um total de 246 pacientes foi submetido à avaliação completa. Metade dos pacientes na amostra relatou história de tabagismo. A proporção de pacientes com resultados anormais na espirometria, radiografia de tórax e SpO2 foi de, respectivamente, 30%, 45% e 13%. Resultados normais em radiografia de tórax, espirometria e SpO2 foram observados simultaneamente em apenas 41% dos pacientes com AR. História de tabagismo foi associada a achados espirométricos anormais, de doença pulmonar obstrutiva e de doença pulmonar restritiva, assim como radiografia de tórax anormal e com padrão intersticial. Na comparação dos pacientes com exames normais (n = 101) com aqueles com exames com alguma alteração (n = 145), houve uma diferença estatisticamente significante entre os dois grupos em relação a idade e história de tabagismo. Interessantemente, sinais de doença de vias aéreas foram observados em quase metade dos pacientes com relato de baixa exposição ao tabagismo ou de nunca ter sido fumante.Conclusões: O comprometimento pulmonar na AR pode ser identificado através de uma combinação de métodos diagnósticos simples, seguros e de baixo custo. Nossos resultados sugerem que pacientes com AR e sinais de acometimento pulmonar devem ser avaliados quanto a possíveis anormalidades pulmonares, mesmo na ausência de sintomas respiratórios.

Introducción: La cánula nasal de alto flujo (CNAF) es un método de soporte respiratorio cada vez más utilizado en pediatría por sus resultados y seguridad. Objetivo: Determinar la efectividad de la CNAF, evaluar factores asociados a fracaso y complicaciones relacionadas con su uso en lactantes. Pacientes y método: Se analizaron los datos demográficos, clínicos, gasométricos, radiológicos y complicaciones de los pacientes conectados a CNAF en una unidad crítica entre junio de 2012 y septiembre de 2014. Se compararon los pacientes que fracasaron con los respondedores a CNAF, considerándose fracaso la necesidad de un mayor soporte respiratorio durante las primeras 48 h de conexión. Se utilizó test de Kolmogorov Smirnov, U de Mann-Whitney, Chi cuadrado, test exacto de Fisher, correlaciones y Modelo de regresión logística binaria para p ≤ 0,05. Resultados: Un total de 109 pacientes. Mediana de edad y peso: 1 mes (0,2-20 meses) y 3,7 kg (2-10 kg); percentil 95: 3,7 meses y 5,7 kg respectivamente. El diagnóstico y patrón radiológico más frecuente fue bronquiolitis (53,2%) e infiltrado intersticial (56%). Un 70,6% respondió. Hubo diferencia significativa entre fracaso y respuesta en el diagnóstico (p = 0,013), radiografía (p = 0,018), contexto de conexión (p < 0,0001), pCO2 (mediana 40,7 mm Hg [15,4-67 mm Hg] versus 47,3 mm Hg [28,6-71,3 mm Hg], p = 0,004) y horas de CNAF (mediana 60,75 h [5-621,5 h] versus 10,5 h [1-29 h], p < 0,0001). El OR de PCO2 ≥ 55 mm Hg para fracaso fue 2,97 (IC 95%: 1,08-8,17; p = 0,035). Ningún paciente falleció ni registró complicaciones. Conclusión: El porcentaje de éxito observado fue similar a lo publicado. En esta muestra el fracaso de CNAF solo se asoció a una pCO2 inicial ≥ 55 mm Hg. Su uso se consideró seguro al no reportarse complicaciones relacionadas a su utilización. Se requiere de un estudio multicéntrico, aleatorizado y controlado para contrastar estos resultados.


Introduction: The high flow nasal cannula (HFNC) is a method of respiratory support that is increasingly being used in paediatrics due to its results and safety. Objective: To determine the efficacy of HFNC, as well as to evaluate the factors related to its failure and complications associated with its use in infants. Patients and method: An analysis was performed on the demographic, clinical, blood gas, and radiological data, as well as the complications of patients connected to a HFNC in a critical care unit between June 2012 and September 2014. A comparison was made between the patients who failed and those who responded to HFNC. A failure was considered as the need for further respiratory support during the first 48 hours of connection. The Kolmogorov Smirnov, Mann-Whitney U, chi squared and the Exact Fisher test were used, as well as correlations and a binary logistic regression model for P ≤ .05. Results: The study included 109 patients, with a median age and weight: 1 month (0.2-20 months) and 3.7 kg (2-10 kg); 95 percentile: 3.7 months and 5.7 kg, respectively. The most frequent diagnosis and radiological pattern was bronchiolitis (53.2%) and interstitial infiltration (56%). Around 70.6% responded. There was a significant difference between failure and response in the diagnosis (P = .013), radiography (P = 018), connection context (P < .0001), pCO2 (median 40.7 mmHg [15.4-67 mmHg] versus 47.3 mmHg [28.6-71.3 mmHg], P = .004) and hours on HFNC (median 60.75 hrs [5-621.5 hrs] versus 10.5 hrs [1-29 hrs], P < .0001). The OR of the PCO2 ≥ 55 mmHg for failure was 2.97 (95% CI; 1.08-8.17; P = .035). No patient died and no complications were recorded. Conclusion: The percentage success observed was similar to that published. In this sample, the failure of HFNC was only associated with an initial pCO2 ≥ 55 mmHg. On there being no complications reported as regards it use, it is considered safe, although a randomised, controlled, multicentre study is required to compare and contrast these results.

ABSTRACT

Fundamento: El proceso de atención médica a los niños con afecciones pulmonares recurrentes en la provincia de Sancti Spíritus, muestra insuficiencias que motivan que esos pacientes no se identifiquen adecuadamente y que existan diferencias en el uso de métodos para el diagnóstico de las posibles causas subyacentes. El diseño de acciones para contrarrestar esas insuficiencias no ha sido descrito aún en artículos científicos. Objetivo: Diseñar una estrategia de intervención en salud para el perfeccionamiento del proceso de atención médica a los niños con afecciones pulmonares recurrentes. Metodología: Se utilizaron los siguientes métodos: análisis documental, observación, entrevistas, grupos nominales, criterios de expertos; así como métodos estadístico-matemáticos. La investigación se dividió en tres etapas y se desarrolló en la provincia de Sancti Spíritus desde el 2009 al 2014. Resultados: Se detectaron insuficiencias metodológicas en el proceso de atención médica a niños con afecciones pulmonares recurrentes. Se elaboró y ejecutó una estrategia de intervención que consistió en el redimensionamiento de la organización existente en los servicios de salud, así como la estructuración de elementos teóricos, clínicos e investigativos para evaluar e investigar esos pacientes. Conclusiones: La estrategia demostró ser efectiva y aplicable; posibilitó una primera aproximación a niños cubanos con afecciones pulmonares recurrentes y permitió elevar el nivel de conocimientos, de los médicos, sobre esas afecciones.


Background: The medical care process to children with recurrent lung affections at Sancti Spíritus province shows insufficiencies that promote inappropriate identification of those patients and use of methods for the diagnosis of the possible underlying causes in different ways. The design of actions to offset those insufficiencies has not still been described in scientific papers. Objective: to design an intervention strategy in health service for the improvement on the medical care process to children with recurrent lung affections. Methodology: The following methods were used: documental analysis, observation, interviews, nominal groups, experts' criteria; as well as statistical-mathematical methods. The investigation was divided into three stages and it was developed at Sancti Spíritus province from 2009 to 2014. Results: Methodological insufficiencies were detected in the medical care process to children with recurrent lung affections. An intervention strategy was elaborated and executed; this strategy consisted on the re dimension of the existent organization in health services, as well as in the structuring of theoretical, clinical and investigative elements to evaluate and to investigate those patients. Conclusions: The strategy demonstrated to be effective and applicable; it facilitated a first approach to Cuban children with recurrent lung affections and allowed to increase the level of doctors’ knowledge on those affections.

The authors reported a lung infection by Rhodococcus equi in a 25 years-old male patient admitted to hospital with cough, dyspnea, fever, and previous diagnosis of pleural effusion. R. equi was isolated from pleural fluid and the patient acquired nosocomial infection by Acinetobacter baumannii, isolated from chest drain. The patient was treated with antibiotics. During hospitalization, he was diagnosed with non-Hodgkin lymphoma of precursor T-cell lymphoblastic lymphoma subtype in biopsy of pleura. After undergoing surgery for pulmonary decortication for drain empyema, the patient died due to septicemia(AU)


Los autores informan de una infección pulmonar por Rhodococcus equi en un paciente masculino de 25 años que fuera hospitalizado con tos seca, disnea, fiebre y diagnóstico previo de derrame pleural. R. equi se aisló del líquido pleural y el paciente adquiere una infección nosocomial con Acinetobacter baumannii aisladas de un drenaje torácico. El paciente recibió tratamiento con antibióticos y, durante la hospitalización, fue diagnosticado linfoma no Hodgkin subtipo de linfoma linfoblástico de precursoras de células T en la biopsia pleural. Después de la cirugía para decorticación pulmonar para la fuga de empiema, el paciente falleció debido a una septicemia(AU)

Objectif : Déterminer les caractéristiques épidémiologiques et cliniques des atteintes pleuro-pulmonaires au cours des connectivites dans l'unité de rhumatologie du CNHU-HKM.Patients et méthodes : Il s'agit d'une étude rétrospective à visée descriptive portant sur des dossiers des patients vus l'unité de rhumatologie dans la période de janvier 2000 à Mars 2013. Dans un premier temps, les dossiers des patients ayant souffert d'une connectivite ont été recensés. Le diagnostic de connectivite a été retenu selon les différents critères diagnostiques TANIMOTO 1995 pour la Dermatopolymyosite (DPM), ACR 1987 pour la Polyarthrite Rhumatoïde (PR), ACR 1997 pour le lupus érythémateux systémique (LES), ACR 1989 pour la sclérodermie, Communauté Européenne 1993 pour le Goujerot-Sjögren, SHARP pour les connectivites mixtes. Dans un deuxième temps, les dossiers des patients ayant souffert de manifestations pleuro-pulmonaires ont été dépouillés. Le diagnostic d'atteinte pulmonaire a été posé sur des arguments clinique, radiologique et/ou après un test thérapeutique. Les données recueillies ont été analysées grâce au logiciel épi info 6.0.Résultats : Sur 9785 patients vus dans l'unité de rhumatologie, 127 cas (1,3%) de connectivites ont été diagnostiqués. Il s'agissait de 99 béninois, 13 nigérians, 5 togolais, 4 ivoiriens, 3 maliennes, 2 burkinabés et une sénégalaise. La sex-ratio était de 0,17. L'âge moyen des patients était de 35,2± 5,2 [18-71] ans. Dix-sept (17) patients ont présenté des manifestations pleuro-pulmonaires. Elles étaient associées à d'autres atteintes séreuses (péricarde et péritoine) dans 6 cas. La toux était présente chez tous les patients. Elle était sèche dans n=14 cas et associée à une dyspnée dans n=5 cas. Les manifestations se présentaient comme suit : pleurésies rattachées au LES et à la PR (n=3 cas chacun), toxicité pulmonaire probablement liée au méthotrexate confirmée par l'arrêt du traitement de fond (n=5), broncho-pneumopathies infectieuses (n=4) et la fibrose pulmonaire en rapport avec la PR (n=2). L'évolution sous traitement (corticothérapie seule ou associée à un traitement de fond) a été favorable chez 14 patients, stationnaire chez 2 patients. Un cas de décès a été noté. Conclusion : Les atteintes pleuro-pulmonaires occupent une place non négligeable parmi les complications liées aux connectivites. La corticothérapie et le traitement de fond par les DMARDs ne sont pas non plus dénudés de complications pulmonaires. La mise en place d'une équipe pluridisciplinaire s'impose pour le dépistage précoce et la prise en charge de ces complications

Previous studies have demonstrated a positive association between obesity and decreased lung function. However, the effect of muscle and fat has not been fully assessed, especially in a healthy elderly population. In this study, we evaluated the impact of low muscle mass (LMM) and LMM with obesity on pulmonary impairment in healthy elderly subjects. Our study used data from the Korea National Health and Nutrition Examination Survey from 2008 to 2011. Men and women aged 65 yr or older were included. Muscle mass was measured by dual-energy X-ray absorptiometry. LMM was defined as two standard deviations below the sex-specific mean for young healthy adults. Obesity was defined as body mass index > or = 25 kg/m2. The prevalence of LMM in individuals aged over 65 was 11.9%. LMM and pulmonary function (forced vital capacity and forced expiratory volume in 1 second) were independently associated after adjusting for age, sex, body mass index, smoking status, alcohol consumption, and frequency of exercise. LMM with obesity was also related to a decrease in pulmonary function. This study revealed that LMM is an independent risk factor of decreased pulmonary function in healthy Korean men and women over 65 yr of age.

Aim: To study the effect of jute-dust and further impact of smoking on pulmonary function test parameters in jute mill-workers. Method and Result: With the help of computerized multifunctional spirometer 91 male workers were compared with 90 controls for their PFT-parameters. The study indicated an overall reduction in FVC, FEV1, PEFR, FEF (25-75) % and MVV. FEV1/FVC was within normal range. Further division of workers into smoker and nonsmokers there was a more decline in FEV1 along with FEV1/ FVC in smokers. Conclusion: Exposure of jute-dust lead to combined type of restrictive/obstructive lung diseases.
